My best guess would be that some of the UO files are broken?

I guess is you restarted and looked if it was gone already, so it is not an unrefreshed client.

I remeber a while back we had the same issues to get rid off the tiles that lap you must patch higher UO Map Files has changed Haven and many other areas as well!
Not sure what Client you have but you should patch in stage's till you log in and see what is suposed to be normal then the tiles will not lap anymore!
I think the patch for running this Run Uo 2.0 is 6.0.6.1 ...and no higher thow or it cuases more problem's
