The best example of this problem is dungeon destard if you go in through the entrance you cannot go back out even after doing a telgen there are no teleporters.
The reason this happens is because of the map changes for KR the teleporters are generating in the ground or whatever and the teleporter.cfg needs an update. |

The teleporters exist at the entrance to get in, but the teleporters don't to get out. The world building files that add the teleporters are missing them for Destard. It's easy to add them. I haven't walked around everywhere though to see if there are any others missing.

There is one missing in doom but that is for a different reason.
The latest changes to doom do away with the old gauntlet secret room door that you could walk right through. Now there is a teleporter that sends you over the wall to prevent monsters from entering the gauntlet secret room. |
