How create new accounts for normal players?

in latest version server monsters is full respawded?

Use the [admin command. In the "Accounts" section simply enter a new name and password and voila... New Account for a player.

You will always have to spawn your own world.

They already answered this. Use [admin when you are logged in your admin account. It's pretty self-explanatory from there.
